New Student Forms

Students who have been accepted and paid their enrollment fee for January 2009, should plan to participate in Orientation on January 6 beginning at 9:00 AM.  Before attending Orientation students should complete each of the following forms that apply to to them.  If you have questions regarding the forms or orientation, please contact the Dean of Students office at 419-448-2062.
